What We Offer

Open, honest and transparent communication with you, which starts with:

Immediate Needs Assessments (INA)

We provide Immediate Needs Assessments (INA) under the 2015 Rehabilitation Code. Once instructed, we contact the client within 48 hours and usually assess within 10 working days.

Reports are delivered within 21 days, including:
• Pre- and post-injury capability overview
• Medical and therapy history
• Goals to help our client recover and feel like themselves again
• Recommendations for short- and long-term rehabilitation 
• Return to work considerations

Ongoing Case Management

We support your clients from discharge to independence:
• Open and trusted communication
• Personalised client approach
• Co-ordinated rehabilitation services
• Liaison with treating teams and family
• Reporting updates aligned with legal processes
• Independent case reviews and proactive problem solving
We operate nationwide, giving you consistent case management wherever your client lives.

Vocational Rehabilitation

• Employer communication and return-to-work planning
• Work site assessments

Outcome-Focused Approach: CM-TOMs

CM-TOMs (Case Management – Therapy Outcome Measures) was developed by Bridge in partnership with Professor Pam Enderby.

Our new innovative tool to the case management industry helps:

• Generate standardised data to improve rehabilitation planning
