Bridge Training Programme & Applications Launching Soon...

Bridge Case Management in collaboration with Prof. Pamela Enderby (Sheffield University) has developed a bespoke outcome measure for case management, as an adapted scale of Prof. Enderby’s well established Therapy Outcome Measure (TOM). TOM is supported by over 40 years of research, validation and evidence, ensuring that CM-TOM is a rigorous and validated tool, with a strong research pedigree. CM-TOM is the only validated service outcome measure for case management.

What is CM-TOMs?

CM-TOMs (Case Management – Therapy Outcome Measures) is a specialist framework developed with Professor Pam Enderby at the University of Sheffield. It helps us consistently assess:
Physical/psychological/cognitive impairment
Independence in daily activities
Participation in social, vocational and education life
Psychological wellbeing
Engagement with rehabilitation

What are the benefits of CM-TOM?

The driving vision behind the creation of CM-TOM was to generate an evidence base for what case managers do, what case management is, and it benefits the clients. With CM-TOM case managers can measure and monitor their clients progress, in a simple, yet holistic manner.
The data will also enable case management services to measure and analyse overall outcomes and trends, which will also provide invaluable information for Stakeholders and funders.
• Identify where progress is happening — or where it's plateauing
• Justify intervention costs with measurable results
• Track and compare outcomes across regions or injury types
• Build stronger, evidence-backed reports for litigation and planning

Maintaining stability in rehabilitation is just as valuable as improvement. CM-TOMs shows both.

Prof. Pam Enderby

Where can I read more about CM-TOM?

CM-TOM is included in the new edition of Prof. Enderby’s books; Therapy Outcome Measure Handbook Theory, User Guide and Scales (2024) and Therapy Outcome Measure: Scales and resources for long covid rehabilitation and case management. You can find the books

How can I get training to use CM-TOMs?

If you are new to TOMs you will need to access the "Introduction to TOMs," training courses provided by Community Therapy Network. This can be done in group training sessions or via the e module (soon to be released). If no training date is upcoming, please let us know and we can request additional training for groups of up to 10. Once you have completed the initial TOMs training, you will be able to access bespoke CM-TOMs training with Bridge Case Management.
• Reliable data across a client’s recovery timeline
• Insight into what interventions work best
• A better understanding of barriers to progress
• Shared language for clients, funders, and legal teams
This training will help you — and your clients — make informed, timely decisions throughout rehabilitation.

We can now match narrative outcomes with objective measures — helping funders, families and professionals alike.

Launching Soon

Why should I do the training?

There are big plans afoot to gather data from across the industry to build a solid evidence base for case management. In order for data to be “gold standard” it needs to be standardised across the industry. This will give us a rich dataset for research and generating the evidence to support not only case managers’ recommendations, but case management as an industry.
If you are interested in learning more about CM TOMs, we are looking at launching our training series very soon, this will be supplemental to the TOMs training provided by Innervate Ltd.
• Intro to TOMs Training via the Community Therapy Network
• CM-TOMs Application Training for case managers (Bridge-specific)
• Group or 1:1 support for those integrating it into practice
