Get Better Understanding • Makes a Real Difference • Life Changing Experience

Case managers don’t enter this profession to tick boxes.

They do it to make the right evidence-based decisions — for the right reasons — for the people who rely on them.

This training shares the learning gained from Bridge’s award-winning CM-TOMs research, so UK case managers can build on what they already know and practise with greater confidence, clarity, and consistency.

I know I’m doing the best I can for my clients.

Gain insight into the latest best-practice tools and techniques to make your Immediate Needs Assessments really make a difference.

Rehabilitation Innovation of the Year

We are extremely proud to announce that Bridge Case Management won the Rehabilitation Innovation Award 2025 at the CMS UK award ceremony on 19th September. The award is in recognition of the case management outcome measure CM TOM — founded by Emma McClean, Clinical Director and Sian Bailey, former Clinical Lead.

Why This Training Matters

This course imparts the research and real-world insights behind CM-TOMs.
Apply outcome measurement with confidence
Make evidence-based decisions in practice
Build stronger, more credible reports
Improve client outcomes with clarity and purpose
Make confident, defendable clinical decisions
Measure Client progress meaningfully

What are the benefits of CM-TOM?

The driving vision behind the creation of CM-TOM was to generate an evidence base for what case managers do, what case management is, and it benefits the clients. With CM-TOM case managers can measure and monitor their clients progress, in a simple, yet holistic manner.
The data will also enable case management services to measure and analyse overall outcomes and trends, which will also provide invaluable information for Stakeholders and funders.
• Identify where progress is happening — or where it's plateauing
• Justify intervention costs with measurable results
• Track and compare outcomes across regions or injury types
• Build stronger, evidence-backed reports for litigation and planning

You'll need to have passed CM-TOMs Introduction Training

If you are new to TOMs you will need to access the "Introduction to TOMs" training courses provided by Community Therapy Network. This can be done in group training sessions or via the e module (soon to be released).

If no training date is upcoming, please let us know and we can request additional training for groups of up to 10.

Once you have completed the initial TOMs training, you will be able to access bespoke CM-TOMs training with Bridge Case Management.
• Reliable data across a client’s recovery timeline
• Insight into what interventions work best
• A better understanding of barriers to progress
• Shared language for clients, funders, and legal teams
This training will help you — and your clients — make informed, timely decisions throughout rehabilitation.

Maintaining stability in rehabilitation is just as valuable as improvement. CM-TOMs shows both.

How the Training Works — Your Step-by-Step Journey

We divide the pathway into clear, achievable stages so you always know what’s next and how each step brings you closer to competence and recognition.

Most participants complete the training in approximately 3–6 months, depending on individual caseload and professional commitments.
This allows time to apply learning properly, reflect on practice and evidence real-world competence rather than rushing through content.
Progress is structured, but flexible — supporting quality, not speed.
