Bridge CM Training

The Best Course of Action • Better Understanding • Make a Real Difference

Case managers don’t enter this profession to tick boxes.

They do it to make the right evidence-based decisions — for the right reasons — for the people who rely on them.

This training shares the learning gained from Bridge Case Management’s award-winning CM-TOM research, so UK case managers can build on what they already know and practise with greater confidence, clarity, and consistency.

I know I’m doing the best I can for my clients.

Gain insight into how CM-TOM can clearly evidence your client’s progress as the only published, validated and ratified case management outcome measure tool.

Rehabilitation Innovation of the Year

We are extremely proud to announce that Bridge Case Management won the Rehabilitation Innovation Award 2025 at the CMS UK award ceremony on 19th September. The award is in recognition of the case management outcome measure CM-TOM — founded by Emma McClean, Clinical Director and Sian Bailey, former Clinical Lead.

Why This Training Matters

This Bridge CM Training course imparts the research and real-world insights behind CM-TOM.
Apply CM-TOM with confidence ensuring you meet the highest standard
Make evidence-based decisions in practice
Build stronger, more credible reports
Improve client outcomes with clarity and purpose
Make confident, defendable clinical decisions
Measure client progress meaningfully

What are the benefits of CM-TOM?

The driving vision behind the creation of CM-TOM was to generate an evidence base for what case managers do, what case management is, and the benefits to the clients.
With CM-TOM, case managers can measure and monitor client progress, in a simple, yet holistic manner.
The data will also enable case management services to measure and analyse overall outcomes and trends, which will also provide invaluable information for Stakeholders and funders.
• Identify where progress is happening — or where it's plateauing
• A better understanding of barriers to progress
• Justify intervention costs with measurable results
• Reliable data across a client’s recovery timeline
• Track and compare outcomes across regions or injury types
• Shared language for clients, funders, and legal teams
• Build stronger, evidence-backed reports for litigation and planning

You'll need to have passed the TOMs Introduction Training

If you are new to TOMs you will need to access the "Introduction to TOMs" training courses provided by Community Therapy Network. This can be done in group training sessions or via the e-module.

If you have completed initial TOMs training, you will be able to access bespoke CM-TOM training with Bridge Case Management.
This training will help you make informed, timely decisions throughout your client's rehabilitation journey.

Maintaining stability in rehabilitation is just as valuable as improvement. CM-TOM shows both.

Where can I read more about CM-TOM?

CM-TOM is included in the new edition of Prof. Enderby’s books; Therapy Outcome Measure Handbook Theory, User Guide and Scales (2024) and Therapy Outcome Measure: Scales and resources for Long Covid rehabilitation and case management. You can find or order the books from good book sellers online or in store.
Course Milestones

How the Training Works — Your Step-by-Step Journey

We divide the pathway into clear, achievable stages so you always know what’s next and how each step brings you closer to competence and recognition.

Time Commitment & Pace

This programme is designed to fit around real casework — not replace it. Most participants complete the training in approximately 6-9 months, depending on individual caseload and professional commitments.
This allows time to apply learning properly, reflect on practice and evidence real-world competence rather than rushing through content.
Progress is structured, but flexible — supporting quality, not speed.

1. Check Eligibility

Proof of passing the Introduction to TOMs course is required to enrol. Click below to access the Intro Training course via Community Therapy Network if you are new to TOMs.
If you have previously completed this you can go straight to the second step.

2. Enrol & Confirm Placement

Choose a date and secure your place — places are limited to support quality learning.

3. Upload Certificate as Evidence of Prior Learning (48-Hour Window )

4. In Theory Learning Session

Attend the scheduled online session with Bridge CM Trainers — a chance to connect, explore CM-TOM foundations and discuss clinical insight.

5. In Practice Phase 1 — Reports

Apply what you’ve learned by completing 10 external reports in your own professional context — at your pace with no time limit. You will need to attend reviews to check accuracy.

6. First Assessor Review

Our assessor reviews your submission and progression. This personalised feedback builds practical confidence.

7. In Practice Phase 2 — Final Assessor Review

When ready, complete another set of 10 external reports and attend the final review stages. An experienced assessor confirms your learnings and application in the field are correct before you progress to our final Bridge CM Training online assessment.

8. Final Assessment — Knowledge Check

This end of the course assessment evaluates your understanding of the CM-TOM principles and practical application. Achieve the pass mark and become CM-TOM accredited.

9. Bridge CM Training Certificate & Digital Badge

Complete all milestones and get awarded our industry recognised certificate celebrating your achievement. You'll also receive a digital badge to add to your email signature and professional online profiles.
